The Assertive Outreach Team (AOT) focuses on individuals with treatment-resistant psychosis, frequent admissions, long inpatient stays, and those with 24-hour support at risk of placement breakdown.
This role aims to build links between internal services, VCSE organisations, and care providers to ensure a multi-agency, collaborative, recovery-oriented approach to complex rehabilitation. It includes fostering a shared ethos, goals, hope, and optimism to deliver person-centred care that promotes independence and community integration.
This role is offered on a permanent basis with part-time hours of 15 hours per week.
Main duties of the Peer Support Worker include:
1. Communicating effectively with a variety of people and maintaining accurate records.
2. Acting as a specialist resource, offering unique perspectives and expertise on recovery resources.
3. Sharing ideas to achieve recovery goals.
4. Co-facilitating group therapy, support, and psycho-education groups within the community.
5. Promoting awareness and understanding through lived experience.
6. Attending Peer Support Group meetings.
7. Supporting individuals and families holistically.
8. Identifying risks and ensuring appropriate assessments are conducted.
9. Participating in support, supervision, and other meetings.
10. Maintaining good communication with colleagues, partner organisations, and external agencies.
11. Collaborating with the Trust social inclusion team and leading quarterly forums with lived experience.
