Job Description
This NHS team is undergoing changes, requiring practitioners to work with a caseload of clients, assessing their mental state face-to-face, and establishing their current needs. The goal is to develop and implement care plans in collaboration with a multidisciplinary team, which includes psychiatrists, CPNs, psychologists, social workers, OTs, and support workers.
The role involves working with clients who are not in crisis but may need support, signposting, or brief intervention before their care is transferred back to primary care services. Knowledge and experience in mental health are required; however, training and support will be provided.
You will manage a caseload of 30+ clients, some with severe mental health conditions, including suicidal tendencies. The work involves collaboration with a wider team to support recovery and stability.

Compensation
Rates up to £14 per hour umbrella.
