What the Job Involves As a member of the Community Resource Team (CRT) Homecare in Cardiff, you will support clients to achieve their reablement and wellbeing goals, working alongside health professionals. You will operate under a service delivery plan, with tasks allocated via a mobile device, working shifts according to a rota. Various rotas and hours are available, including evening and weekend work. You will play a pivotal role in preventing unnecessary hospital admissions and facilitating hospital discharges, enabling clients to live independently at home.
