About The Role
We are seeking Senior Social Care Assessors to provide community-based adult social care within Herefordshire.
You will primarily work in city settings, utilizing your knowledge and experience in fast-paced teams.
As a Senior Social Care Assessor, you will collaborate with occupational therapists, partners, social care assessors, and social workers to undertake strengths-based assessments, mental capacity assessments, care management, and reviews for a diverse group of adults and informal carers.
A driving license is essential, as travel is required for this role.
About You
You will have:
* An NVQ3 qualification or higher in a related field, or equivalent experience, along with GCSEs in English and Maths.
* Experience working in a care environment.
* Knowledge of current legislation and guidance impacting adult social care.
* Experience in person-centred, strengths-based, outcome-focused assessments, care management, and reviews.
* Ability to support individuals in identifying goals and developing strength-based plans.
* Good relationship-building skills with service users, families, carers, colleagues, and external organizations.
