An exciting opportunity has arisen for a Registered Nurse Associate or Assistant Practitioner to join the East Lancashire District Nursing Team. This role is ideal for someone who enjoys working both autonomously and as part of a wider supportive team, delivering high-quality planned care within the community.

Successful candidates will be able to demonstrate strong problem-solving skills, adaptability and resilience at interview. You will be supported in your development through a dedicated mentor and a comprehensive competency framework, ensuring you feel confident and secure in your role.

This is a 7-day service, including weekends and Bank Holidays, and the ability to work contracted hours on a rotational basis across day and night shifts is essential to meet service needs. A full UK driving licence and access to a vehicle are also required for this community-based role.

Main duties of the job

The post holder will deliver high-quality clinical nursing care to housebound patients, including end-of-life care, wound management, and support for people with complex long‑term conditions. Working closely with colleagues, patients, families, and carers is essential to ensure care is holistic, person‑centred, and responsive to changing needs.

They will review planned care and accelerate any changes or concerns to registered nursing staff in a timely manner. Strong communication skills are vital, as the role requires effective relationships with clinical teams, patients, families, and other professionals.

The post holder will contribute to service development by supporting initiatives that improve patient care, while also taking responsibility for their own professional development to maintain high‑quality practice.

They must demonstrate professional standards, behaviours, and values in line with the NMC Code and the organisation's expectations. We are seeking someone passionate about excellent patient care, able to problem-solve and adapt to change, confident in seeking support when needed, and committed to evidence-based practice and a career in District Nursing.

* Full UK driving Licence, with access to a vehicle is essential


Qualifications

* Level 5 Foundation Degree in Health & Social Care (Assistant Practitioner)/Registerd Nurse Associate Qualification
* Evidence of recent appropriate study/development and application in the work environment


Experience

* A minimum of 12 months experience working in a health care environment


Knowledge and Skills

* Ability to establish and maintain effective communication with patients and informal carers about care / interventions
* Ability to work within a multidisciplinary team
* Evidence of clinical skills relevant to long term & complex conditions
* A working appreciation of Confidentiality, Health, Safety & Security, Risk Assessment, PDR & Supervision, Accountability
* Ability to complete records accurately in accordance with policies and procedures
