What You Will Do:
* Conduct thorough assessments to identify functional limitations and determine individual care needs
* Deliver services according to the Care Act 2014 legislation ensuring comprehensive care act reviews are completed
* Collaborate effectively within a multidisciplinary team to ensure the best outcomes for clients
* Apply your strong knowledge of safeguarding principles and the Mental Capacity Act to safeguard vulnerable adults
* Develop creative interventions tailored to the unique needs of each individual
* Communicate clearly and effectively with clients, families, and colleagues to facilitate excellent care
* Manage your caseload efficiently using strong organisational and time management skills
