Enderley Road Medical Centre has an excellent opportunity for an experienced Advanced Nurse Practitioner to join our friendly, supportive and forward thinking Training practice. This role will be a part time role on Mondays 9am to 6pm for 8.5 hours per week.


Main duties of the job

The Advanced Nurse practitioner will be responsible for a number of clinical areasfocusing mainly on seeing patients for minor ailments, either face to face and over the phone and is able to prescribe. The Advanced nurse practitioner must have experience working in a GP setting and be able to assess, diagnose and treat the patient.


About us

The practice has a registered population of 12,300 patients with 5 partners, 4 salaried GPs as well as an extensive clinical team comprising of Practice Nurses, Health Care Assistants, Phlebotomist, PCN Pharmacist, PCN Physician Associate, PCN First Contact Practitioner and supported by a large team of receptionists and administrators. The practice is also an established training practice with GP Registrars, Foundation Doctors and Trainee nurse. The clinical system used by the practice is EMIS web and has a CQC rating of Good. The practice is part of the Healthsense Primary Care Network (PCN).

* You will be entitled to 5 weeks paid holiday per calendar year excluding bank holidays.
* Weekly MDT Meetings are held at the Practice.


Job responsibilities

* Managepatients presenting with a range of acute and chronic medical conditions,providing subject matter expert advice to patients over the phone and face to face

Providewound care (ulcer /Doppler etc.) to patients

Undertakethe collection of pathological specimens

Request pathology services as necessary

Process and interpretpathology and other test results as required

Providechronic disease clinics, delivering patient care as necessary, referring patientsto secondary / specialist care as required

Maintainaccurate clinical records in conjunction with extant legislation

* Ensureread codes are used effectively
* Supportthe team in dealing with clinical emergencies
* Recognise,assess and refer patients presenting with mental health needs

Extendedand supplementary prescriber, adhering to extant guidance

Supportpatients in the use of their prescribed medicines or over the counter medicines(within own scope of practice), reviewing annually as required

* Contributeto practice targets (QOF etc.), complying with local and regional guidance
* Supportand participate in shared learning within the practice
* Participate in thereview of significant and near-miss events applying a structured approach i.e.root cause analysis (RCA)


Person Specification


Qualifications

* Post graduate diploma or degree (Advanced Practice Qualification)
* Qualified Triage Nurse
* Teaching Qualification
* ALS, PALS


Experience

* Experience of working in a primary care environment
* Experience of working as a practice nurse or community nurse
* Experience of prescribing and undertaking medication reviews
* Experience of chronic disease management


Skills

* Excellent communication skills (written and oral)

* Competent in the use of Office and Outlook

* Requesting pathology tests and processing the results, advising patients accordingly

* Immunisations (routine, childhood and travel)
* Understands the importance of evidence based practice
* Broad knowledge of clinical governance
* Ability to record accurate clinical notes
* Ability to work within own scope of practice and understanding when to refer to GPs
* Knowledge of health promotion strategies
* Understands the requirement for PGDs and associated policy
* Knowledge of public health issues in the local area
* Awareness of issues within the wider health arena
