NIGHTINGALE HOUSE HOSPICE OUTPATIENT LEAD £44,780 per annum 37.5 hours per week We are an ambitious organisation providing specialist palliative care services, completely free-of-charge, to patients and their families across a wide area stretching from Wrexham, Flintshire, and East Denbighshire to Barmouth and the border towns including Oswestry and Whitchurch. It costs £5.6m per year to run the Hospice, with only 20% of our funding provided by the government. We are reliant on the support of the community we serve to donate and fundraise to maintain our vital services. Over 400 volunteers give their time to the Hospice helping us run efficiently and effectively as part of the local community. At Nightingale House Hospice, we believe in making every moment matter. The successful candidate will be joining a friendly and passionate team that enjoys working in our Wellbeing Centre. We are looking for an exceptional Outpatient Lead to join our dedicated multi-disciplinary team providing high standards of holistic care to our patients and their families under the direction of the Clinical Director to our patients living with life limiting conditions. You will work as part of the team to provide holistic person-centred care, integrating nursing care with psychological, physical, rehabilitative and spiritual needs. You need to be able to demonstrate evidence-based practice, up to date clinical skills, flexibility and a commitment to the delivery of high quality, effective and efficient nursing care. You will have experience of managing a department including staff and programme management. Key responsibilities include: Lead and coordinate the Wellbeing Team. Oversee the daily operations of the outpatients and wellbeing service, ensuring high standards of care and a positive patient experience. Support service planning, pathway development, and improvements in Wellbeing. Liaise closely with the Patient Flow Team and clinical staff to ensure safe and efficient operations. Support the inpatient unit during periods of need to ensure continuity of care. Conduct clinical observations, assessments, and monitoring aligned with hospice procedures and within professional boundaries. Undertake holistic assessments contributing to care planning, including physical, emotional, social, and spiritual needs. Support the wellbeing and rehabilitation programme, contributing to group and one-to-one patient interventions. Escalate any clinical concerns promptly to theappropriate medicalor senior nursing staff. Contribute to triage, patient flow management, and safe decision making within the outpatient service.
