Due to increased demand, we are looking for a Health Care Assistant/Phlebotomist to join our friendly Practice family. Oswald Medical Centre is an award-winning practice serving over 16,000 patients. We are based across 3 sites in Accrington, Oswaldtwistle, and Blackburn. We pride ourselves on providing caring services and offer a range of NHS and enhanced services, including minor surgery.

We are seeking a competent Health Care Assistant/Phlebotomist who can work independently and as part of a team. The successful applicant should have previous experience working in a busy GP surgery, carrying out a variety of duties to meet patient needs.


Main duties of the job

* Provide high-quality care based on the needs of the Practice population, including treatment, screening, preventative care, and patient education.
* Assist GPs/PN/NA in various tasks.
* Manage patients with chronic disease, run specific clinic sessions, and support call and recall systems.
* Venepuncture and other skill-based tasks.
* Conduct home visits for housebound patients to manage long-term conditions.
* Assist with minor surgery using aseptic technique.
* Identify unmet needs of patients with LTC, including housebound patients.
* Perform NHS health checks.
* Function as a chaperone when required.
* Undertake health promotion and patient education.
* Weighing and measuring patients.
* Undertake opportunistic screening.
* Update care plans and communicate with all involved in patient care.
* Provide individual advice on smoking cessation, healthy eating, and refer to in-house staff and services.
* Develop and maintain health promotion materials and notice boards.
* Refer patients to other agencies as appropriate.
* Apply infection control measures according to guidelines and support colleagues in infection prevention.
* Safeguarding adults and children.
* Be competent in anaphylaxis and resuscitation techniques.
* Assist with flu clinics.
* Perform 24-hour ECGs after training.

Job responsibilities

* Maintain accurate patient records and enter data using SNOMED codes.
* Ensure proper documentation of patient care.
* Participate in audits and research projects.
* Contribute to practice development plans.
* Recognize and refer appropriately for issues like family violence, substance use, and addictive behaviors.
* General
* Maintain a safe and therapeutic environment.
* Ensure compliance with health and safety regulations.
* Promote a professional appearance and attitude.
* Maintain professional development and PDP.
* Attend staff and clinical meetings.
* Perform other duties as required.


Person Specification


Experience

* Experience in a primary care nursing team.
* Experience working with vulnerable patients.
* Experience in a GP practice.


Qualifications

* HCA & Phlebotomy qualification or equivalent.
* Training in chronic disease management.
* GCSE level or above.
* Experience with long-term conditions.
* Experience working in primary care.
* Time management skills.
* Professional, confident, and polite.
* Ability to plan, organize, and perform under pressure.
* Use of initiative and flexibility.
* Confidentiality and knowledge of clinical governance.
* Experience with health promotion and EMIS Web.
* Understanding of the QOF and clinical audits.
