Overview
As part of our Service Redesign, new posts have been developed to support our high risk patients who require a fast response in order to prevent hospital admissions where possible. This is an exciting opportunity for an innovative and experienced therapist to join the countywide Worcestershire Adult Speech and Language Therapy (SLT) Service, which covers Community Hospitals, Out-patient Clinics and Specialist Teams, with the aim of providing the optimum evidence-based service for the people of Worcestershire.
Full time fixed term 12 month Countywide post (part time or job share considered; minimum 0.4 WTE) will focus on Fast Response under the guidance of a Highly Specialist Clinical Lead, as well as working alongside experienced community SLT colleagues. Clinically, you will provide specialist assessment, diagnosis and case management for patients with a range of communication disorders and dysphagia related to a variety of diagnoses and may maintain a small caseload.
Should part time hours be required, it may be possible to split the post to accommodate this, with 0.6 WTE for Fast response including a session for triage, and 0.4 WTE for outpatient clinics and home visits, depending on the interests and experience of the applicants.
This is a fixed term post to provide cover for absence for specific purpose that we anticipate will be required for 12 months. We reserve the right to end this contract sooner with appropriate notice.

Responsibilities
You will work in a clinic, hospital or domiciliary setting to provide assessment, diagnosis and full case management of adult patients with communication and swallowing difficulties.
Following your patient assessment, you will be responsible for providing appropriate intervention and evaluating outcomes.
You will communicate complex condition-related information to patients, carers and relevant colleagues, as well as demonstrating practical procedures where appropriate.
You will demonstrate clinical knowledge and effectiveness by the use of evidence-based practice and outcome measures.
You will demonstrate commitment to personal development and acquisition of further skills and knowledge.
You will adhere to National and Local standards and guidelines relating to professional practice.

Person Specification
Qualifications
* Attainment of degree or equivalent, giving eligibility to practice as a Speech & Language Therapist
* Evidence of recent CPD relevant to this post
* Attainment of formal dysphagia competencies allowing independent assessment, treatment and full case management with this client group
* Attainment of RCSLT NQP Competencies Framework allowing full membership of RCSLT
* Current HCPC Registration
* Current RCSLT Membership
* I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
Knowledge
* Current knowledge of assessment, treatment and case management for adult clients with complex communication disorders and dysphagia
* Awareness and understanding of current national, local, professional and clinical initiatives relevant to this post
* Demonstrable knowledge of the work of relevant organisations within the voluntary sector in the support of adults with acquired communication disorders and/or dysphagia
Skills & Abilities
* Ability to work well in a team but also to use own initiative

Experience
* Experience of assessment, diagnosis and full case management of adults with communication disorders and dysphagia, in a range of settings
