Job Summary
Working in accordance with the Nursing and Midwifery Council (NMC) Code of Conduct, the post-holder will be responsible for delivering high‑quality nursing care, including patient assessment, treatment, screening, health promotion, education and clinical advice. The role involves delivering nursing care independently and collaboratively with nursing colleagues and General Practitioners to ensure safe, effective and patient‑centred care.

Responsibilities
* Management of chronic disease
o Identifying significant abnormalities
o Diagnosis, monitoring and development of individual management plans; agreeing these as appropriate with patients and other professionals
* Therapeutic monitoring
o Checking compliance and adherence to treatments using a holistic patient‑centred approach
o Recognising abnormalities
o Identifying impact of treatment and implementing or recommending changes
* Wound Care and Management
o Assessing routine wounds including trauma
o Assessing and prescribing to support good wound management in complex situations (e.g. use of Doppler technique)
* Patient Health Checks
o Identifying significant abnormalities
o Obtaining appropriate information using a lifestyle questioning approach and identifying health promotion issues
o Working with patients to develop a management plan where health problems or potential health problems are identified
* Risk Assessment
o Recognising issues and gathering sufficient information to refer (e.g. drugs, domestic violence, child protection, vulnerable adults, senior patients, social problems)
* Health Screening
o Recognising issues and gathering sufficient information to refer (e.g. women's and men's health, sexual health, older people)
o Undertaking monitoring tasks (e.g. smears) and providing advice as appropriate
o Providing in‑depth monitoring and advice as appropriate to the level of the post‑holder's specialty and working with the patient in deciding on management plans
Qualifications
* Registered General Nurse (RGN) with current registration with the NMC and proven experience in a General Practice environment.
* Qualified at diploma level or above, with demonstrable experience across a broad range of primary care nursing services (post‑natal checks, diabetic reviews, cervical cytology, coil and ring pessary procedures, compression bandaging, wound care, Zoladex, immunisations, ECG, blood‑pressure monitoring, vascular clinics, menopause support, catheter care, Doppler studies, family planning, sexual health).
* Ongoing commitment to continuous professional development, clinical excellence and delivery of high‑quality, patient‑centred care within a multidisciplinary primary care environment.
Experience
* Minimum of three years experience working within a Practice Nurse role in a General Practice or primary care setting.
* Evidence of ongoing CPD activity and up‑to‑date clinical knowledge, skills and competencies relevant to the role.
* Ability to critically evaluate safety, effectiveness and quality of own practice in accordance with professional standards and evidence‑based practice.
Desirable Experience
* Effective training and educational skills; ability to support, guide and share knowledge with colleagues, patients and the multidisciplinary team.
* Substantial General Practice experience, including management of chronic disease, and ability to deliver evidence‑based, patient‑centred care and support effective management of long‑term conditions.
