About the Role
You will work collaboratively with secondary mental health services, including Primary Care Mental Health Link Practitioners, primary care services, and the Voluntary and Private Sector, to provide a Recovery-focused model of care.

Requirements
Due to the nature of the work, applicants must be a car driver and own a car.
Key Responsibilities
* Work as a valued member of the Multi-Disciplinary Team under supervision of community mental health practitioners
* Promote and monitor physical and mental health of service users
* Provide person-centered, holistic care to empower service users and support their recovery goals
* Support service users in engaging with community resources, developing life skills, and building support networks
* Assist with physical health checks and promote proactive health management
* Detect, alert, and respond to safeguarding concerns in line with policies
