Overview
We have a vacancy in Hillingdon for a full-time Band 5 Community Staff Nurse in the School Nursing Service. The School Nursing Service is part of the 0-19 service in Hillingdon and is recognised as delivering an innovative and progressive service. The borough is diverse, with strong partnership working with schools, children centres, family hubs and other agencies. Clinical Responsibilities
* Deliver the Healthy Child Programme.
* Undertake holistic health needs assessments of children in a wide range of environments and across a broad population spectrum; interpret, analyse and collate complex information, liaising with children, young people and families, locality School Nurse and other services/agencies as appropriate.
* Implement a plan of care using evidence-based criteria; monitor progress and adapt plans to maximise health benefits for clients.
* Provide skilled, evidence-based nursing care in line with policies and procedures.
* Provide ongoing support for individuals and families, sometimes delegated by senior staff, reporting back to senior colleagues.
* Signpost and refer to other specialist services for support and intervention.
* Assist children, parents and carers to identify their knowledge gaps and offer information and advice to enable access to services and adoption of healthy lifestyles.
* Communicate effectively with children, parents/carers and other professionals.
* Maintain accurate and comprehensive records in line with NMC guidance and Trust policy.
* Demonstrate knowledge of information sharing and assessment protocols and identify relevant information to promote child safety.
Professional Responsibilities
* Communicate with consideration of culture, ethnicity, language, disabilities, developmental stage and environmental settings.
* Engage with children and families, social care, education, colleagues and other professionals using excellent communication skills.
* Manage records in line with Trust policies and guidelines; ensure competence with IT systems used within CNWL (e.g., Outlook, Systmone).
* Maintain Infection Control procedures and implement recommendations as necessary.
* Participate in team discussions and support service developments.
* Maintain revalidation and up-to-date NMC registration; adhere to the NMC Code of Conduct.
* Reflect on own performance through clinical supervision and participate in 1:1 meetings and appraisals; pursue ongoing professional development and personal objectives.
* Complete mandatory training annually or as required.
* Be aware of safeguarding policy and procedure; complete mandatory safeguarding training, attend safeguarding meetings and supervision, and report concerns as needed.
* Monitor own practice to maintain specialist knowledge; seek support as needed and contribute to a high-performing team.
* Support colleagues, including less experienced staff, and work autonomously as well as within a team.
Person Specification
* Essential: Current NMC registration for Nursing (Adult, Mental Health or Child Branch) or due to qualify; numeracy skills for safe administration of medicines; ability to communicate with the multidisciplinary team; maintain accurate records; professional, registered nurse in line with the NMC Code of Conduct; ability to work as part of a team and with sensitivity and assertiveness as needed; work well with others and be reliable and enthusiastic.
* Desirable: Teaching or mentoring skills; evidence of CPD; experience of safeguarding children, health promotion, and group facilitation.
Experience
* Essential: Experience of working in multidisciplinary teams; experience as a Band 5 nurse or student nurse; ability to work with minimal supervision and manage own caseload; experience in managing staff and handling difficult situations.
* Desirable: Previous community nursing experience as Band 5 or equivalent, experience of safeguarding children, health promotion, and group facilitation.
Qualifications
* Essential: Current NMC Registration (Adult, Mental Health or Child) or near qualification; evidence of relevant CPD.
* Desirable: Teaching or mentoring qualifications or equivalent.
Other
* Travel to various sites within the Trust is essential; access to a car or other vehicle for work purposes; full UK Driving License; ability to work effectively in the community.
