Join to apply for the Diabetes Prevention Practitioner - North East London role at Thrive Tribe

Join us on our journey to empower over a million people to change their behaviours and thrive!

At Thrive Tribe we deliver award-winning healthy lifestyle and wellbeing programmes that make people happier and healthier. It’s a really exciting time to join us as we continue to deliver the Heathier You NHS Diabetes Prevention Programme throughout North East London and we’re looking for someone to join our team of passionate health coaches (or Diabetes Prevention Practitioners)!

The purpose of a health coach is to improve lives, through empowering people to make effective long-term changes that halt or reverse the progression of Type 2 Diabetes. This is achieved through successfully delivering our quality programmes and providing evidence-based information, supporting individuals to improve their health, lose weight and ultimately lower their risk of developing diabetes.

Like all roles at Thrive Tribe, you can expect your time at work to be fun, varied and challenging. We don’t like to stick to a firm JD as we know from experience that things change throughout the lifetime of a role and it gives our people the chance to play to their strengths.

A typical day as a Diabetes Prevention Practitioner might include :

* Delivering face-to-face evidence-based interventions that focus on diet, nutrition, physical activity and behaviour change
* Facilitating group intervention sessions to support your clients in achieving their goals and cultivating a safe and positive environment
* Calling patients who have been referred into the programme to book them into upcoming groups
* Engaging in ongoing training relevant to your role, including submitting your annual CPD portfolio and evidencing professional development in line with our CPD policy
* Working towards criteria and targets established within the National Diabetes Prevention Programmes (NDPP) quality assurance framework, including all Clinical Governance procedures
* Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments – we’re always looking to share knowledge and resources!
* Ensuring due consideration and adherence to our internal Thrive Tribe policies, including Information Governance, Safeguarding and Complaints procedures
* Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values!

As a key aspect of your role, you will be responsible for transporting equipment, such as scales to and from various community venues.

About You…

More than anything, we are looking for a team player who puts their heart in to their work. We’ve got some core values that run through everything we do, and we’d love it if they resonate with you too!

Be the change

You like helping people. You’ll be working with those who need help the most, empowering them to make positive changes to thrive.

Be effective

You’re patient and can adapt under pressure. Always happy to make time for other people, and you have a great attitude.

Be happy

You recognise the power of a smile. You value a fun, supportive workplace and want to come to work raring to go!

Be successful

You are motivated and driven. Happy working solo or as part of a team, you love meeting targets. And will be given opportunities to contribute and develop your career.

Be big-hearted

You want to make a difference. Working with a like-minded team of optimistic people, you to do meaningful work and change lives.

We’re looking for someone who can combine health improvement knowledge with first class communication and motivational skills. If you have experience of delivering group-based interventions, and a real understanding of the impact personal characteristics and circumstances can have on health behaviours then we want to hear from you. Bonus points if you have a qualification in a health promotion related subject, such as nutrition, exercise, or health coaching!

We hire based on attitude over traditional skills and experience so if you don’t have all of the necessary experience but feel you are right for the role we would love to hear from you!

Perks…

Every person makes Thrive Tribe the incredible place it is to work and play together, we’re unstoppable. Within our buzzing environment our talented people work hard but we ensure lots of fun is thrown in too! We offer you;

* Regular team events and activities, including ThriveFest and our annual Challenge Weekend (for those who want to put their fitness to the test!)
* Flexible working – we’re serious about life/work balance
* Tailored health and wellbeing support such as access to life coaching
* Rewards portal offering discounts on shops, restaurants, and days out
* Enhanced parental leave
* 23 days holiday, increasing every year up to a maximum of 29 days (plus Bank Holidays!)
* And last but not least, a day on us to live by our values!

Contract: 1 x Permanent, Full Time (37.5 hours) position available

Salary: £24,000 per year (rising to £27,000, with development opportunities to earn up to £31,000 as a Senior Health Coach)

Applicants will be contacted to outline the next stages of the process. Interviews are likely to take place in early July.

To apply: Please attach your CV and a covering letter outlining why you are right for the position along with which boroughs you would be happy to deliver group sessions in: -

* Barking and Dagenham
* City of London
* Hackney
* Havering
* Newham
* Redbridge
* Tower Hamlets
* Waltham Forest

Location: This is a hybrid-remote role. You will need to live in North East London as the role requires regular travel across North East London along with some working from home.
