We are looking for a dedicated Senior Mental Health Liaison Worker to join our Community Mental Health Service in Somerset. This is an exciting opportunity to play a key role in transforming how mental health care is delivered across primary and secondary services. You will work within neighbourhood teams and Primary Care Networks, providing early assessment and intervention for adults experiencing mental and emotional distress, including those with complex needs and older adults.
The role focuses on breaking down traditional barriers between services, ensuring continuity of care, and promoting best practice in mental health support. You will collaborate closely with GPs, health professionals, and voluntary sector partners, while also offering guidance and clinical supervision to colleagues.
