Job Summary
Overview
The Quality and Safety Facilitator Midwife is a role that is integral to promoting and supporting a positive culture of learning. It provides essential support to the Maternity and Neonatal team through co-ordination of clinical risk across the service on a daily basis. The individual will work with all grades of clinical staff and managers across the multi-disciplinary team (MDT) to identify risks, report risks appropriately, be involved in the incident reporting and management system, ensuring required actions follow and learning takes place. The individual will contribute to the healthcare governance agenda with the Quality and Safety Team.
Responsibilities
* To support the provision of healthcare to women and new-borns by supporting risk management and patient safety incident mechanisms.
* To receive, review, grade and investigate all reported clinical incidents and near misses with divisional and central risk management teams.
* To ensure immediate actions from incidents are escalated as appropriate.
* To ensure effective written and verbal communication with all staff within maternity services, provide feedback, and signpost staff to appropriate support services.
* To maintain an incident reporting system fulfilling internal and external reporting requirements, ensuring investigations are conducted and improvements evidenced.
* To collate and maintain a database of incident data and use trend analysis to identify patterns, triangulate information, and implement mitigations to reduce risk.
* To promote a Just Culture approach within patient safety incidents.
* To lead by example and share expertise in clinical practice.
Qualifications
Essential
* NMC Registered Midwife
* 3 years of recent NHS clinical experience in acute and community midwifery settings
* Datix (incident reporting) skills
* Awareness of clinical governance and NHS Safety Framework
* Evidence of facilitating learning
* Knowledge of NHS Just Culture
* IT skills
Desirable
* Evidence of continued professional development
