Our clinicalpharmacy service is growing and we are looking for a pharmacist to support safeuse of medications within our organisation.
The postholder will provide leadership on medicines management and optimisation, andquality improvement and manage some aspects of the quality and outcomesframework.

Main duties of the job
To providecomprehensive medicines information and advisory service to other healthcareprofessionals in order to ensure the best individualised choice of medicationfor patients and avoid delays in treatment.
To ensureparity of esteem is prioritised and that the physical health of patients ismaintained and monitored. To provide care plans to help improve physical healthfurther and liaise with organisations who can support physical healthimprovements.
Work as partof the multi-disciplinary team (MDT), liaising with other pharmacy teammembers, nurses, doctors and other healthcare professions in order to resolvepatient centred pharmaceutical problems.
Toreduce the likelihood of service user (re)admissions resulting from medicationrelated issues. To provide support to the teams around safe discharge toPrimary Care and care planning. The purpose of this role will be to providecomprehensive clinical pharmacy service to inpatient and community teams.

Job responsibilities
The purposeof this role will be to provide comprehensive clinical pharmacy service toinpatient and community teams, ensuring high quality, safe, effective, legaland accurate use of medicines within each speciality.
Thesuccessful candidate will support the development of better medicinesmanagement understanding throughout the organisation and promote good practicein line with National and local guidelines. They will support the ChiefPharmacist in maintaining and improving the pharmaceutical service to Navigoand be responsible for the delivery of an agreed standard of pharmaceuticalcare to identified patients in order to achieve the safe and cost effectiveuse. They will support patients and carers to gain maximum benefit from theirmedicines through medicines related consultations. Experience of working withina hospital pharmacy department is desirable, although applications frompharmacists working within the community sector and newly qualified pharmacistsare welcome. The successful candidate will share our values and have a positiveattitude, skills and enthusiasm to contribute to shaping how we support ourservice users. The ability to travel independently is essential as visits toother external sites will be required. Person Specification
Qualifications
* Statutory registration with the General Pharmaceutical Council, UK.
* Clinical or psychiatric postgraduate diploma (for band 7).
* Membership of the Royal Pharmaceutical Society, UK.
Experience
* Completed Pharmacy Foundation training
* Suitable number of years post-registration experience as a practising pharmacist (for band 7).
* Experience in working in a mental health hospital pharmacy environment or similar.
* Experience in leading audits, projects or research facilitation and management (for band 7).
* Experience of service planning and development.
* Experience of established Multi-disciplinary Team working.
* Experience of providing patient information and therapeutic engagement.
* Experience of teaching and training HCPs at a range of levels (undergraduate and postgraduate).
* Experience of managing unpredictable work patterns and to effectively manage situations where concentration may be frequently interrupted.
