About The Role We are looking for a dedicated and compassionate Dementia Adviser to join Alzheimer's Society in Wakefield. This role focuses on triaging new referrals to our service, acting as the first point of contact for individuals and their families who need our support. You will be part of a collaborative and supportive team based at the Wakefield Connecting Care Hub, playing a crucial role in helping people navigate the complexities of dementia. This is an exciting opportunity to make a real difference in the lives of those affected by dementia, providing essential advice and guidance at some of their most challenging times. Interviews will be held week commencing 23rd June About you: We are looking for someone with strong communication and interpersonal skills who is confident engaging with individuals affected by dementia and their families. Ideally, you will have experience in a support, advisory, or triage role, with a background in health, social care, or the voluntary sector being highly desirable. You will be able to approach sensitive situations with empathy, offering support and reassurance. The ideal candidate will be proactive, organised, and able to work both independently and as part of a close-knit team. Familiarity with using IT systems to manage referrals and record case notes will also be important. What you'll focus on: In this role, your primary focus will be to assess the needs of individuals referred to the service and ensure they receive the appropriate support. You will be responsible for providing empathetic and individualised information, advice, and support, as well as signposting to other services where necessary. By being the first point of contact, you will help ensure that people affected by dementia in Wakefield are connected to the right services in a timely and effective manner. Your work will contribute to improving access to care and enhancing the experience of those impacted by dementia, ensuring they feel supported and informed throughout their journey.
