Job Responsibilities
* Work closely with clinical and admin teams to ensure the efficient processing of repeat prescriptions including medication monitoring for high‑risk drugs.
* Monitor and act on drug safety alerts, medication reconciliations for discharge and clinical letters.
* Collaborate with the Practice Pharmacist(s) on medicines optimisation and patient care activities.
* Assist in the continuous development of the service while maintaining high standards and best practice.

* Medicines management experience in a ward‑based setting (medication history taking, medicines reconciliation)
