Overview
As Lead PCN Mental Health Practitioner, you’ll transform community mental health services across GP practices in Central Norwich, delivering patient clinics with support, medication advice (if qualified to prescribe), treatment, education and problem-solving approaches for people with mental health challenges. You’ll shape the service and manage a small team of Mental Health Practitioners in a Place Based Care approach with Integrated Care System partners.
Responsibilities
* Lead a small team of Mental Health Practitioners within allocated GP Practice(s).
* Set up and deliver patient clinics providing assessment, treatment, education and problem-solving for mental health needs.
* Provide medication advice where qualified to prescribe.
* Operate an intervention-led approach; provide high-quality care and make clinical judgments.
* Coordinate with primary and secondary care services to bridge gaps between services.
* Work autonomously with age 16+ patients; conduct Mental Health Assessments and refer to specialist services when required.
* Manage day-to-day operational and clinical issues, working at pace in a GP setting.
Qualifications and experience
* Registered mental health professional (e.g., Mental Health Nurse, Occupational Therapist, Social Worker) or other registered professional with Mental Health experience.
* Experience working in a mental health setting and ability to work autonomously and as part of a team.
* Strong organisational skills, resource awareness, and ability to build relationships with healthcare professionals.
* Autonomous practitioner capable of clinical judgment and alternative action planning.
Working arrangements
We are happy to accept full-time or part-time applications and can be flexible regarding working patterns within GP practice hours and enhanced hours.
