The Role
As Care Coordinator, you will play a central role in ensuring safe, high‑quality, person‑centred care across our services. You will work closely with operational and quality teams, offering clinical leadership and supporting best practice in line with CQC standards.
Key Responsibilities
* Provide clinical leadership across supported living services.
* Review and implement care plans for individuals with complex or high‑risk needs.
* Act as the lead clinical liaison for referrals, transitions, and MDT meetings.
* Support service managers to embed best practice and maintain CQC compliance.
* Monitor incident reports, safeguarding alerts, and health outcomes.
* Lead on health audits, clinical risk assessments, and medication reviews.
* Train and mentor support staff in trauma‑informed, high‑quality care.
* Work collaboratively with local authorities, NHS commissioners, and families.
What We’re Looking For
Essential
* NMC Registered Nurse (RMN, RNLD, or RGN with community/complex needs experience).
* Minimum 3 years’ post‑registration experience in a clinical or care‑coordination role.
* Experience in supported living, residential care, or community adult health.
* Strong understanding of CQC, DoLS, MCA, and risk management frameworks.
* Excellent communication, leadership, and organisational skills.
* Ability to travel between services (mileage reimbursed).
Highly Desirable
* Experience with forensic clients or dual diagnosis.
* Experience supporting services through CQC inspections.
