Are you a Physiotherapist looking for a fantastic community opportunity where you'll be supporting people back to or within their own homes?
We are seeking a Physiotherapist to join our friendly D2A service, part of the wider Somerset Intermediate Care service. This role offers in-depth experience in community working and supporting individuals in their homes.
Due to positive outcomes and growth, we have secured further investment to expand our service and are recruiting for several new positions.

Main duties of the job
You will conduct specific therapy assessments, including goal setting, treatment planning, risk assessment, and promoting independent functioning. A person-centered approach, prioritization skills, and flexibility are essential, working alongside provider teams and rehabilitation practitioners.
