Pay £13.21 per hour (£25,838 per annum) and great benefits including Health Cash Plan

What you’ll do
6. Creating support plans with your customers and coordinating our small team of Support Workers working to the plans.
7. Supporting 16-24 year old customers in an OFSTED service
8. Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.
9. Carrying out risk assessments, support planning, goal setting and regular reviews.
10. Cuppas, chats and catch ups. Plenty of time to collaborate with colleagues.

You have
11. Passion to support our customers to live their best life, working collaboratively with an eye for detail.
12. Experience of creating person-centred support plans and supporting colleagues in working to work to the plans.
13. Experience of coordinating and assessing customer referrals.
14. The ability to work on your own initiative, remain calm under pressure and have a resilient approach

The practical bits
15. Where we’re flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
16. You will work on a 4 week rolling rota, with evening and weekend working. You will do 37.5 hours a week.
17. Although you’ll lone work, we do get together regularly [in the daytime] for training and team meetings.
18. Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
19. You’ll need an Enhanced DBS check done and we pay for that.
