REACT Occupational Therapist / Physiotherapist

We are seeking a passionate and motivated Occupational Therapist / Physiotherapist to join our Rapid Emergency Assessment and Care Team (REACT), an essential part of UHCW's Emergency Medicine Group. This 12-month position is to cover maternity leave, with potential for internal secondment.
REACT operates within Emergency Medicine's Assessment Areas and Short Stay wards, aiming to prevent hospital admissions while delivering high standards of care and discharge planning.
Our team is committed to safely discharging patients into the community through comprehensive holistic assessments and collaboration with external community partners.
The team comprises Occupational Therapists, Physiotherapists, and Assistant Practitioners. We value team success and prioritize staff wellbeing, offering regular supervision, in-service training, and opportunities for professional development. Successful candidates will contribute to supervision and team development.
With a focus on integrated care, there are opportunities for service development involvement.
Our service operates 7 days a week with extended hours, offering various shift patterns, including long days and weekends.
Main duties of the job
1. Manage a diverse caseload of patients with varying conditions and needs, using evidence-based, client-centered practices to assess, plan, implement, and evaluate interventions.
2. Identify and utilize appropriate discharge pathways to promote a 'home first' approach and support patient flow.
3. Lead clinical areas across the Emergency and Acute Medicine footprint, providing daily support to the team.
4. Support the learning and development of junior staff and students.
5. Participate in planning, developing, and evaluating the REACT service, including managing specific projects.
6. Work within a 7-day service model, including extended hours.

Job responsibilities
1. Be professionally and legally responsible & accountable for all aspects of your work.
2. Manage a clinical caseload with patients presenting varied conditions, complexities, and social needs.
3. Conduct holistic assessments to determine treatment and discharge plans, ensuring patient-centered decisions.
4. Collaborate with patients, families, carers, and external services to prevent unnecessary hospital stays and support timely discharges.
5. Work autonomously within a multidisciplinary team, contributing to discussions and decision-making.
6. Implement therapeutic interventions using a range of modalities, applying clinical reasoning to select appropriate techniques to improve function and reduce deconditioning.
7. Evaluate and adjust therapeutic and discharge plans as patient needs change.
8. Identify and prescribe necessary equipment, conducting risk assessments as needed.
9. Accurately document all clinical activities in accordance with REACT policies.
