We are working with a well-established, forward-thinking GP practice that is seeking an enthusiastic and committed Practice Nurse to join their friendly multidisciplinary team. Key Responsibilities
* Provide high-quality nursing care across treatment room and chronic disease management clinics
* Undertake immunisations, cervical cytology, wound care, ear care, health checks and long-term condition reviews
* Support management of diabetes, COPD, asthma, hypertension and other long-term conditions within scope of competence
* Deliver health promotion, lifestyle advice and patient education
* Work autonomously while contributing to a wider multidisciplinary team
* Maintain accurate clinical documentation in line with practice protocols and professional standards
* Participate in safeguarding, infection control and clinical governance activity
* Contribute to quality improvement initiatives and the ongoing development of clinical pathways
Requirements
* Registered Nurse (NMC) with primary care experience
* Confident in core Practice Nurse skills (immunisations, cytology, wound care, long-term condition monitoring)
* Strong communication and patient-engagement abilities
* Ability to work both independently and collaboratively within a GP practice environment
* A positive, proactive approach with a commitment to delivering safe, high-quality care
* Additional chronic disease qualifications or experience are highly desirable
