We have restructured our Adult Social Care teams in Cambridgeshire and invite you to join our Adults Community Teams as an Adult Support Coordinator. We have a vacancy in our Cambridge and South Cambridgeshire Team, based in Cambridge. We embrace new ways of working, allowing our social work teams to work where they need to. You'll be part of a collaborative team environment with strong working relationships with health professionals and partners to achieve better outcomes for the adults we support. Responsibilities
You will liaise with other departments, undertake Social Care and Mental Capacity Act assessments, and plan care and support to achieve better outcomes for the adults we support.
