Harrow Community Mental Health Triage and Assessment Team, based in Harrow and Wealdstone, is looking for a passionate and experienced RMN who is looking to make a difference whilst working in an environment that promotes staff development and training opportunities.

Main duties of the job
To be you and to do what you do best in the job you love. You will be accountable for the RMN intervention of a defined caseload of adults. To assess health and social care needs (including accommodation, finances, and support in accessing education, training or employment) and negotiate care plans with service users and their carers, remaining objective when working with diagnostic uncertainty.

Detailed Job Description and Main Responsibilities
The post will involve responsibilities under the following key areas:
Clinical
* Receive referrals from GPs and undertake comprehensive psychosocial mental health assessments to establish care needs.
* Act as a liaison with GP practices for patients with complex mental health problems and/or co-morbidities.
* Screen non-urgent GP Adult mental health referrals to identify patients suitable for primary care management and reflect local resource thresholds.
* Facilitate onward referral and rapid access to mental health services and other agencies, or advise referring GPs on effective management.
* Ensure early detection and fast tracking of clients with severe mental health problems.
* Provide appropriate short-term interventions for clients with more complex and severe, enduring mental illnesses (SMI).
* Support safe discharge case management of patients discharged from secondary care, enabling GP practices to provide continuing care and access to wider social and self-management supports.
* Refer-on patients with common mental health problems to primary care psychological therapy pathways or external resources.
* Facilitate onward transfer of suitable clients from secondary care to primary care where necessary.
* Develop and maintain a high standard of mental health professional care using a liaison model of nursing with a problem-solving and evaluation approach.
* Utilise a range of therapeutic techniques and be aware of social factors affecting mental health, respecting diverse cultures and beliefs.
* Receive regular supervision as agreed with the Mental Health Triage Team Manager.
* Support the work of the depot clinic if required as part of a rota.
* Undertake risk assessments according to Trust policy.
Communication and Liaison
* Provide mental health advice and support to GP practices for management and care of clients with mental health difficulties.
* Act as liaison between primary and secondary care for clients known to mental health services; liaise with mental health professionals, social services, and other agencies.
* Participate in Mental Health Triage professional team meetings.
* Recommend and facilitate admission for clients when clinically indicated.
* Identify conditions affecting wellbeing and refer to appropriate agencies.
* Establish and maintain working relationships with individuals and agencies in mental health.
* Provide written reports about clients to referrers and other relevant agencies.
Person specification
Qualifications
Essential criteria
* Current Professional Qualification in Mental Health Nursing.
* Registration must be live with UK NMC professional body.
* Evidence of post registration training and CPD.
Desirable criteria
* Training in Psychosocial Interventions or Thorn Family Therapy/Work training
* Other relevant training, e.g. psychosocial interventions
Experience
Essential criteria
* Experience of working in multidisciplinary mental health teams with clients with acute/severe and enduring mental health problems.
* Extensive post-registration experience.
Desirable criteria
* Work with clients who are homeless/rootless or experiencing crisis
* Supervision of students and junior staff
* Audit and research
* Working with people who use substances

Skills
Essential criteria
* Excellent verbal and written communication skills, including report writing and IT skills.
* Ability to assess, plan, implement and evaluate a comprehensive care package for the identified client group using a wide range of resources.
* Ability to develop a comprehensive programme of intervention and support for the identified client group and their families.
* Ability to carry out a comprehensive assessment and make autonomous plans.
Desirable criteria
* Leadership skills: ability to motivate others, use initiative and manage change.
* Basic counseling and group work skills.
