Youwill guide our patients through their crucial initial phase post-transplant.You'll be at the forefront, providing expert care and support during theirinpatient journey. The post holder is responsible for:
Empowering patients with vitalbedside ERAS training, setting them on the path to a smoother recovery.
Facilitating seamless transitionshome through comprehensive discharge education, ensuring they feel confidentand prepared.
Collaborating closely with themultidisciplinary team during ward rounds, contributing your specialistknowledge to optimise patient care.
You'llalso play a key role in our busy early transplant clinics, where you'll havethe opportunity to build relationships and provide ongoing support topatients as they navigate their new lives post-transplant.
Main duties of the job
Thejob holder will be instrumental in managing the inpatient workload, includingdaily ward visits, delivering vital bedside ERAS training and support,providing comprehensive discharge education, and actively participating inward rounds. Their focused attention during this early stage is paramount tofacilitating timely and well-informed discharges.
Furthermore,this role extends to the early transplant clinics, which run twice weekly andtypically involve reviewing up to 6 patients each session. Currently, thesubstantive CNS in this role manages an average of 30 patient reviews perweek. The job holder will work closely with the wider transplantmultidisciplinary team including specialist pharmacists, nephrologists andconsultants. The job holder will have the opportunity to engage withtransplant education sessions and local transplant network meetings tounderstand the wider transplant picture with focus on service development.

TheLeeds Renal Services lies within the Abdominal Medicine Surgery (AMS)Clinical Service unit at Leeds Teaching Hospital Trust.
Themain renal unit is at St James's University Hospital, where inpatient,outpatient, pre-dialysis, haemodialysis, peritoneal dialysis andtransplantation services are provided. Nurse-led outpatient clinics also takeplace at St James's.

JOBPURPOSE
Thejobholder will be joining an established team of clinical nurse specialistswho provide care to a caseload of approximately 1250 post renal transplantpatients. They will play a key role in the initial post-transplant phasesupporting their inpatient journey and will then go on to support them thepatients in the early transplant clinic. Here they will have the opportunityto support the patient as they navigate their new lives post-transplant.
Thejob holder will join an established specialist nursing team who provide careand advice to a specific patient group and will contribute to the developmentof the speciality service.

Experience
* Substantial experience at Band 5 or equivalent including experience within a relevant specialty area during which time the following experience should have been gained
* Clinical audit and standard setting
