Overview
The Community Rehabilitation and Home First teams are part of the Penwith Integrated Community Team. They include registered and non-registered nursing, therapy and administrative staff, working with partner organisations to support the intermediate and rehabilitation needs of the local population. This position is open to those who wish to develop assessment, care planning, reablement and rehabilitation skills whilst being part of a dynamic, research-active, progressive, integrated, multiagency team. This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.
We offer a supportive working environment with a structured in-service training programme; appraisal and supervision system. This is a full-time role with a 7-day rota that may include some weekends and bank holidays; flexible working arrangements may be discussed.
This role is open to current NHS Cornwall or Isles of Scilly staff only.
Main duties
* Responsible for assessment, planning and implementation of care; interpret information relevant to the patient’s condition/progress; identify problems and needs; set realistic rehabilitation goals; provide specialist evidence-based treatment plans for adults in their homes or care homes.
* Collaborate with wider multidisciplinary teams within the community and liaise with Community and Acute Hospitals, Adult Social Care, and Voluntary Services.
* Cover the Penwith area (Hayle to Marazion and surrounding West Cornwall); participate in a 7-day rota and flexible working hours (8am–6pm).

Person specification
Essential criteria
* Degree in occupational therapy
* HCPC registered
Desirable criteria
* Experience of working in a community setting
Skills and Aptitude
* Skills in clinical assessment, intervention and evaluation
* Ability to engage people with significant communication impairments
* Caseload management with supervision and prioritisation
* Willingness to work with people who have behaviours that challenge
