An exciting opportunity has arisen to join the Warrington Recovery Team and help establish a new role within our Service.
Warrington Recovery Team (Warrington Adult Community Hub) is looking to recruit a B6 Community Mental Health Transition Practitioner to support the transition for young adults aged 18-25 into the CMHT/Recovery Team. The role involves connecting young adults to their chosen pathways and interventions, engaging with the young adult, carer, and referrer prior to referral where possible, and supporting informed decisions around treatment and transition.
The Transition Practitioner will aim to meet NICE transition standards and promote Mersey Care principles of transition. They will be an integral part of the Recovery Team MDT, working closely with Children and Young People services and other supporting services across the wider system, including primary care and VCFSEs.
Our office base is Wakefield House, with core hours currently from Monday to Friday, 9-5 (subject to change). The role requires initiative and flexibility to support all potential referrals of young adults aged 18-25, supporting them until they achieve their transition goals and begin treatment within the team. The practitioner will act as a keyworker during this period, helping navigate the mental health system and serving as a point of contact for the young adult and their carer.
The Transition Practitioner will regularly analyze performance against transition standards to identify areas for quality improvement and help shape the role across Community Mental Health Services.
