We have a vacancy within the Community Nursing Networks in North Lincolnshire.
As a District Nurse you will carry a caseload of complex patients and contribute to the management of the teams.
Main duties of the job
Band 6 District Nurses will join the forward‑thinking District Nursing teams as part of the integrated Health and Social Care teams within the community setting. The role requires a sound understanding of the current nursing and health agenda, excellent communication and leadership skills, and the ability to work autonomously while promoting a positive learning environment for team members and students.
* Update and populate rosters, manage daily sickness, support the workforce, and complete projects as required.
* Take forward the expanding role of community nursing within the multi‑disciplinary integrated team to meet long‑term conditions and end‑of‑life care agendas.
* Link and work closely with GP’s; demonstrate commitment to modernising community nursing; use effective communication and negotiation skills.
* Gain caseload management skills and experience during a 6‑month preceptorship period (newly qualified District Nurses).

Person Specification
* Education and Qualifications
o Specialist Practitioner Qualification in District Nursing
o Nurse Prescribing
o Evidence of further professional development.
o One year post‑degree clinical experience.
* Occupational Experience
o Evidence of leadership, change and people management.
o Evidence of commitment to continued professional development.
o Experience of change and people management.
o Experience of research/development and audit.
* Disclosure and Barring Service Check – Post subject to Rehabilitation of Offenders Act (Exceptions Order) 1975; submission to DBS/CRB required.
Salary
£39,959 to £48,117 a year pro rata
