Main duties of the job
This job is including face2face to discuss medication issues with patients as needed and appropriate, carry out medication reviews and chronic disease medicines management. Prescribing medication following prescribing systems and policies. Liaison with community and hospital pharmacies. Medication reconciliation and medicines information according to guideline.

Job responsibilities
The Practice Pharmacist prescriber is an integral part of the clinical team, using and sharing pharmacy prescriber expertise to support the GPs and the whole practice in effective medicines management and optimal patient care. To develop the role of Practice Pharmacist prescriber in close collaboration with the partners.
Person Specification
Experience
* Have relevant clinical experience.
* Experience of working in GP surgery.
Qualifications
* Must be registered with the General Pharmaceutical Council (GPhC), have relevant clinical experience and complete an accredited independent prescribing course.
* Must be registered with the GPhC and completed the prescribing course.
