An exciting opportunity has arisen to work within HBCCC/ICF (Hospital Based Complex Clinical Care/Interim Care Facility) Hospital Social Work Team as a Social Worker. We are looking for highly motivated and enthusiastic Social Workers to join our fast-paced and dynamic service which provides a responsive and person-centred service in supporting timely and safe discharges for adults from non-acute hospitals facilities. Where an individual has spent a continuous period of 3 months or more out with the UK in the last 5 years, an Overseas Criminal Record Check will be required. You will be required to provide this check.
