An exciting opportunity has arisen for a Nurse to join our friendly Primary Care Team.
We have approximately 12,000 patients on our list and we are based in St Helens town centre.
Main duties of the job
Working in accordance with the NMC Code of Conduct, the post-holder will provide assessment, treatment, screening, health education services and advice to patients. The post holder will have the required level of training and competence and will work within patient group directions where these are available.
Responsibilities include: chronic disease management, hypertension management, vaccination and immunisation (including children and travel), cervical cytology screening, INR monitoring, family planning advice, venepuncture, removal of sutures, wound care, health promotion, delivery of the annual influenza vaccination programme, assistance with GP duties, simple procedures, independent decision-making or recommendations alongside the GP.
A full, clean driving licence and use of a car are essential.
Key responsibilities
* Chronic disease management
o Identifying significant abnormalities
o Diagnosis, monitoring and developing individual management plans; agreeing these as appropriate with the patient and other health professionals
* Therapeutic monitoring
o Checking compliance with appropriate treatments using a holistic patient‑centred approach

o Identifying the impact of treatment and implementing or recommending changes as appropriate
* Wound care and management
o Assessing routine wounds including trauma
o Supporting good wound management in complex situations (e.g., use of Doppler technique)

o Working with patients to develop a management plan where health problems or potential health problems are identified

o Recognising issues and gathering sufficient information to refer (e.g., drugs, domestic violence, child protection, vulnerable adults, senior patients, social problems)

o Recognising issues and gathering sufficient information to refer (e.g., women and men’s health, sexual health, older people)

Person specification
Qualifications
* First level Registered General Nurse (with current live registration)
* 2 years desirable experience of General Practice Nursing
* Knowledge of asthma, diabetes, well‑person (including cervical cytology), immunisations, health promotion, and minor surgery
* Working knowledge of general practice policies and procedures, scope of professional practice
Experience
* Experience of working within a nursing team in a primary care setting
* A full driving licence and use of a car
* Strong communication, information technology, time management, and organisational skills
* Team working and self‑motivation
* Experience with vulnerable patients and family planning, chronic disease management, well‑person programmes
