Are you passionate about reshaping mental health care? We're launching an innovative community care centre inspired by world-class models like the Trieste Mental Health Community Network, Navigo in Grimsby, and Clubhouse models in Brixton. We have an exciting and rare opportunity for an exceptional mental health pharmacist to join the pilot in Lewisham. This centre will hold a major national profile with NHS England, aiming to radically transform mental health service delivery. The ideal candidate will have experience with front-line teams and share the ethos and outcomes of the pilot. The pilot is expected to run for two years, starting in 2025.
The key features include:
* A focus on people's life stories, needs, and aspirations, as determined by them and their networks.
* A wider range of purposeful interventions, including building trust with professionals.
* More therapies, improved medication management, and social interventions such as employment, leisure, substance issues, and physical health care.
* Addressing inequalities for Black communities in mental health services, with an aspiration to eliminate waiting times through an 'open access' model.
* Enhanced options to support those nearing crisis and providing a 'softer landing' for individuals experiencing crisis.
Main duties of the job
The pilot aims to improve quality of life indicators, reduce hospitalisations, and minimize coercive practices.
Job Purpose:
* To provide an expert pharmacy service to the Lewisham community mental health hub.
* To collaborate with MDTs to ensure effective and appropriate medication use.
* To optimise psychiatric medication use with MDT support.
* To improve physical health outcomes for community mental health patients, including vaccination, monitoring, and medicines optimisation.
* To liaise with primary care, acute trusts, HTTs, and wards as needed.
* To offer medication consultation clinics for patients and carers.
* To run group medication information sessions for patients and carers.
* To implement medicines optimisation programs within teams.
* To undertake research, audits, and quality improvement projects.
* To provide medicines-related training to community staff.
* To prescribe medicines within competence and management plans.
* To deprescribe medicines within competence and plans.
* To optimise medicines use, reduce wastage, and maximize resource efficiency.
* To record and analyse data for outcome measures.
