As the discharge coordinator, you will work in the ward environment alongside the multidisciplinary team to ensure effective discharge planning and communication, providing a smooth and timely discharge from hospital for all patients.

Excellent written and verbal communication skills are essential, along with the ability to work autonomously and as part of a team. Professionalism, interpersonal skills, and the ability to prioritize workload by recognizing simple or complex needs are crucial. You will coordinate the discharge or transfer process with the multidisciplinary team.

Experience with hospital IT systems is desirable but not essential, as full training and support will be provided. You will be responsible for updating Trust IT systems (Medway/Nerve Centre, etc.) regarding bed states and discharge planning, working alongside bed managers to ensure timely patient transfers and discharges to maintain patient flow.
