We have an exciting opportunity for experienced Social Workers to join the Mental Health Social Care Team in the vibrant London Borough of Ealing on a full-time basis for three months. The position involves working with a dedicated team to assess and review the social care needs of adults in Ealing with primary mental health needs.

What you will do:
* Undertake assessments and reviews in line with the Care Act for adults with mental health needs.
* Develop person-centred, strengths-based support plans.
* Conduct capacity assessments under the Mental Capacity Act.
* Compile Social Circumstances Tribunal reports.
* Act as a Social Supervisor for conditionally discharged patients.
* Perform safeguarding enquiries and work collaboratively with the Safeguarding Adults Manager.
* Participate in the duty system to screen referrals and address queries.
The ideal candidate will have a strong grasp of the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983 (2007), and Human Rights Act 1998, with a flair for professional curiosity and a strength-based approach. Experience in dealing with Court of Protection Schedule 21A challenges will be beneficial.
