Job Description:

NHS Greater Glasgow and Clyde is one of the largest healthcare systems in the UK employing around 40,000 staff in a wide range of clinical and non-clinical professions and job roles. We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.

The shift pattern for this position is 5 days over 7 including weekends and public holidays. May also include the out of hours period.

Caring is fundamental to nursing services and to all nursing functions. The core values of nursing must underpin the practice of every nurse and drive care that promotes positive and equitable engagement with patients, families and carers as the central focus of practice. The community nursing service operates 24 hours per day providing care to patients within their own homes. Community nursing services offers a wide range of skilled clinical interventions which encompass a broad spectrum of technical expertise, health education and health promotion in order to maximise the people of our community’s independence and support carers. The community nursing service works collaboratively and flexibly with all disciplines to support the people who live in Inverclyde.

The job the applicant will be doing:

As an Advanced Nurse Practitioner and specialist practitioner (District Nurse), you will support the people within our community who require advanced nursing care within their own homes. You will work with the district nursing teams and other community nursing teams supporting the transformation of nursing and excellence in care agendas. You will play a pivotal role in supporting modernisation of community nursing

services within the Health and Social Care Partnership and provide the clinical experience necessary to support care delivery in the right place at the right time. You will participate and lead in key working groups where education and development of nursing staff may be required.

The post holder will be the clinical expert who will be accountable for case management within the caseload, and/or lead, manage and support the District Nursing team.

The post holder will be expected to provide an accessible, appropriate, high quality, culturally sensitive, safe, person centred and effective service in the community, by working in partnership with people and families, care providers and other agencies.

The post holder will function as an independent, autonomous practitioner by virtue of in-depth knowledge, expertise, proficiency and experience to lead and enable their team to provide a safe, person centred and effective quality service.

The district nurse / advanced nurse practitioner will work collaboratively with all team members to provide effective and holistic nursing care to the people within our community.

The post holder will work collaboratively in both a planned and responsive manner to ensure nursing skill and expertise is afforded to residents of Inverclyde HSCP.

There will be a requirement to link & liaise effectively with GPs and out of hours medical / nursing colleagues, Scottish Ambulance Service and provide guidance and support to unregistered staff supplying care & support services to HSCP service users.

Skills / Qualifications:

A 1st level registered nurse with significant post registration experience is essential. Specialist practitioner qualification in district nursing and advanced nurse practitioner is required. Community experience is essential.

An interest in Palliative Care and Management of Long Term Conditions / Anticipatory Care alongside associated technical skill & ability is considered advantageous. E.g. enteral feeding T34 Syringe Pumps

You should therefore note that candidates who are unable to provide confirmation of current NMC registration, in line with projected start date for this post, may not be shortlisted for interview. This post requires PVG Scheme Membership / Disclosure Scotland Check. Full driving licence essential.
