Essential criteria
* NMC Registration - Registered 1st level nurse or HCPC Paramedic Registration
* Recent external CPD, post-qualification
Desirable criteria
* Specialist courses appropriate to Primary care e.g. CHD, Diabetes, Respiratory conditions, minor illness and minor injury.
* Complex Case Management (LTC) qualification or experience
* Relevant postgraduate study at masters level
* Unwell Adult Clinical Skills Qualification
* Minor Illness Qualification
* External Leadership Qualification
Experience / Knowledge
Essential criteria
* Evidence of relevant experience working in a health care setting at senior clinical level
* Diagnostic skills qualification or appropriate experience
* Demonstrable experience of line management
Desirable criteria
* Experience in recording patient-related information electronically and in written form, in both office and home settings
* Experience working with community development initiatives
* Knowledge of change management tools and ability to influence and implement change
