Clinical Fellow in Hospital at Home
We are pleased to offer the opportunity for a Clinical Fellow doctor (post FY2+) to join our team in the Hospital at Home service from December to March 2026 for a fixed term of 4 months. The post will provide an excellent opportunity to develop skills in acute medicine, realistic medicine and comprehensive geriatric assessment in a community setting. This would be an ideal opportunity for someone who has completed Foundation Training and is interested in pursuing a future career in Medicine of the Elderly, another medical specialty or General Practice. Applicants should be within four years of qualification but more senior applicants may be considered.
The Hospital at Home service aims to manage acutely unwell patients in their own home either via admission avoidance pathways or facilitated discharge. It provides specialist hospital-level care for frail older adults aged 65 years and older in their familiar surroundings. It is a multidisciplinary consultant-led service with close links to community therapy staff. Care is delivered by a combination of video virtual ward rounds and comprehensive assessment and treatment delivery within the home environment. The service works across NHS Lanarkshire from four base hubs: Clydesdale, University Hospital Monklands, University Hospital Hairmyres and University Hospital Wishaw. The post will be based at University Hospital Hairmyres to expand their already established service. Flexibility may be needed to provide medical cover for the other areas as the service requires would be essential.
The Hospital at Home service is committed to providing an enjoyable and rewarding working experience, where there are excellent opportunities for service development. Hospital at Home is an area that is growing rapidly and we have a passion for delivering excellent services to look after acutely unwell patients in their own home.
The post does not have educational approval for it to be recognised for training.
