Job summary The post holder will provide leadership as a Specialist Community Co-Occurring Practitioner working closely with both Community Mental Health Teams (CMHT) and Substance Misuse Services (SMS) in local County based services. Main duties of the job The post holder will work with the MDT to lead on the management and monitoring of the Co-Occurring Framework Pathway across both Mental Health and Substance Misuse services. The Post holder will have responsibility to maintain a clinical caseload. The post holders' role will include setting and maintaining clinical standards within the governance framework and the motivation and leadership of team members from both CMHT and SMS services in reaching specific clinical, health and Co-Occurring framework targets. The post holder will be responsible for developing good relationships between services, both statutory and third sector to promote the wellbeing of individual service users and their families. The post holder will conduct and coordinate comprehensive assessments of need to formulate effective care plans for the benefit of those service users and their families as well as agreed service standards and outcome measures. The post holder will provide clinical teaching and education to other professionals in order to enhance clinical practice and improve care provision. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
