Croydon serves a residential population of approximately 370,000, with 27% of these being within 0-18 years of age. Over the last 5 years, emergency admissions for children and young people with asthma and wheezing have risen and are now the second highest reason for hospital admissions in Croydon. Currently, 3,751 children aged 0-17 have a diagnosis of asthma.
As part of the paediatric asthma nurse specialist team, you will work alongside other asthma nurse specialists to deliver and maintain optimal treatment plans for children with asthma in Croydon. You will also collaborate with other paediatric areas such as Rainbow Children's Unit, Paediatric Emergency Department, and other nurse specialists. The primary goal of the service is to improve asthma management outcomes by increasing confidence and self-management skills among children, young people, and their carers. Most interventions will be conducted as single, 1-on-1 visits focusing on prevention through education and effective self-care, rather than case management.
The post holder will work across community, primary, and acute care settings as an expert in this clinical field, providing a specialist asthma service to children, young people, and their families. Facilitating multi-professional working and education across all sectors is essential.
The service operates Monday to Friday, with hours scheduled to meet the needs of the population. The post involves 37.5 hours per week.
Key responsibilities include:
1. Organising and planning the day-to-day management of the Children's Asthma team, ensuring individual needs are met.
2. Demonstrating flexibility in work hours and locations to multitask effectively.
3. Identifying and managing risks to patients and staff.
4. Developing and delivering innovative self-management education approaches for different age groups.
5. Disseminating best practices in asthma management to community clinicians.
6. Collaborating with GP surgeries to ensure treatment aligns with best practices.
7. Working with Public Health to identify and reach hard-to-engage groups, focusing on health outcomes and reducing inequalities.
8. Providing line management, support, and mentoring to colleagues.
9. Developing and monitoring quality initiatives to ensure high standards of nursing care, aligned with the CHS clinical governance framework.
10. Working strategically to influence service development.
