What you will do
* Deliver high-quality, skilled nursing care to patients in their homes.
* Provide rapid response, rehabilitative, and post‑acute care across community settings.
* Work shifts that span from 8 am to 10 pm, 7 days a week, including weekends and bank holidays.
* Utilize your specialist skills to holistically assess patient needs and develop short‑term care programmes based on evidence‑based practice.
* Focus on service priorities, such as admissions avoidance and timely supported discharge.
* Manage your caseload and support your team with the guidance of the Team Leader.
* Supplement locality‑based planned care teams and collaborate with other health services when acute or intensive care is needed.
* Actively engage in service development activities to enhance care delivery.
Requirements
* RGN qualification and NMC registration with a valid live pin.
* Recent UK nursing experience, particularly in Community or District Nursing.
