Recovery Coordinator

Salary: £24,000 per annum, plus benefits

Working within an integrated healthcare framework, the role involves supporting drug and alcohol users aged 18 and over (dependent on service). The services aim to deliver psychosocial drug and alcohol interventions, providing client-centered treatment using harm reduction and abstinence approaches based on individual needs. Responsibilities include managing caseloads, offering harm reduction advice, conducting assessments, care planning, and delivering structured 1:1 and group sessions, including structured treatment programs. Collaboration with services such as Housing, Employment, Healthcare, Probation, and CRCs is essential. The role also involves contributing to targets set by Forward and local commissioners, with a focus on measuring outcomes through cooperative relationships with partner agencies. Flexibility for evening/weekend shifts with reasonable notice is required to ensure continuity of care.
Strategy
* Participate actively in implementing and achieving team objectives.
* Build strong relationships with line management and head office support.
Service Delivery
* Conduct risk and comprehensive assessments to inform treatment pathways.
* Deliver interventions tailored to client needs, including 1:1 and group work.
* Coordinate with other agencies, including training, employment, and family support services, to meet client needs.
* Work alongside healthcare providers to monitor and manage client health and risks.
* Develop and contribute to assessment and care plans, supporting engagement with specialist agencies.
* Motivate clients to engage with support services and support their care plans, including referrals and advice on housing, benefits, and employment.
Performance Management
* Ensure facilitation of groups and programs meets required standards.
* Achieve KPIs by set deadlines.
* Manage caseload effectively, prioritizing clients appropriately.
* Review ongoing care and liaise with healthcare and other agencies.
* Utilize supervision for reflection and personal development.
Departmental Management
* Support integrated working and multidisciplinary approaches.
* Meet contractual targets and contribute to smooth service operation.
Quality and Safety
* Engage in service audits and address issues promptly.
* Report risks and adhere to policies and procedures.
* Promote health, safety, and security, reporting risks as per protocols.
* Attend safeguarding training and report safeguarding concerns.
* Manage data and information securely and in compliance with legal standards.
Systems and Policy
* Utilize IT tools effectively.
* Share information appropriately within protocols.
* Use available systems like Illy, Intranet, and email.
Administration
* Maintain case management documentation to audit standards.
* Submit data forms timely for outcome reporting.
* Review and update care plans regularly.
