Overview
Calderdale Council is seeking experienced Qualified Social Workers (QSWs) to join our Hospital Discharge Team and Post-Discharge Assessment Team. This is an exciting opportunity to work in a fast-paced, supportive environment where you will play a key role in facilitating safe and timely hospital discharges, and ensuring continuity of care within the community.
Responsibilities
* Undertake hospital discharge assessments for patients at Huddersfield Royal Infirmary and Calderdale Royal Hospital.
* Work collaboratively with health professionals, patients, and families to plan and coordinate safe discharge arrangements.
* Where required, provide cover for the Post-Discharge Assessment Team, assessing and reviewing individuals discharged into community settings – whether returning home or moving into care placements.
* Ensure compliance with the Care Act 2014, Mental Capacity Act 2005, and other relevant legislation and policy frameworks.
* Maintain accurate and timely case records and documentation.
Requirements
* Qualified Social Worker (Degree in Social Work, CQSW, DipSW or equivalent).
* Current registration with Social Work England.
* Experience within hospital discharge, adults, or community social work teams.
* Strong assessment, decision-making, and communication skills.
* Ability to work effectively within a multi-disciplinary team.
* Enhanced DBS check required.
