<div><h3>RRIC Community Practitioner - Physiotherapist</h3><p>Our innovative Rapid Response & Intermediate Care service is expanding across Buckinghamshire and we need you to join our Thame and Amersham Teams working across Intermediate Care and Community Physiotherapy.</p><h3>Job Responsibilities</h3><ul><li>Provide triage, comprehensive specialist assessment, review and evaluation of the needs of patients and carers.</li><li>Plan programmes of care to promote health gain and maximise independence, actively case manage patients through their journeys.</li><li>Allocate appropriate workloads to members of the Rapid Response & Intermediate Care (RRIC) team.</li><li>Work with a range of professionals to ensure care is appropriate and delivered in a timely manner and demonstrate professional skills and competence in leadership, care planning/coordination, care and treatment delivery.</li><li>Support the development of clinical pathways.</li><li>Work across Intermediate care and Community Physiotherapy pathways.</li></ul><h3>Person Specification</h3><h3>Education, Qualifications & Training</h3><ul><li>Registered Healthcare Professional with HCPC Physiotherapist</li><li>Degree/Diploma in Physiotherapy</li><li>Experience of recent Continuous Professional Development</li><li>Relevant postgraduate clinical training e.g. Advanced History Taking & Assessment</li><li>Long Term Conditions: Respiratory Disease, Cardiac Disease, Trauma, Orthopaedics & End of Life</li></ul><h3>Experience</h3><ul><li>Demonstrate experience post or preregistration placements in relevant clinical areas for therapists: Care of Elderly, Orthopaedics, Neurology, Respiratory, Palliative Care, Long term conditions</li><li>Experience of working in an NHS community/in‑patient setting for a minimum of 2 years post registration</li><li>In depth experience of working within multi‑agency/multi‑disciplinary team</li><li>Experience of working with NHS or Adult Social Care single assessment process</li><li>Experience of electronic clinical systems</li><li>Experience participating in research or audit projects</li><li>Experience of day‑to‑day staff management e.g. case allocation to Health Care Assistants</li></ul><h3>Skills, Abilities & Knowledge</h3><ul><li>Demonstrate profession specific clinical assessment, planning, delivery and evaluation skills</li><li>Ability to motivate staff</li><li>Excellent written and verbal communication skills</li><li>Ability to manage complex cases</li><li>Ability to develop and maintain partnership working</li><li>Knowledge of legal requirements for working with vulnerable adults and those who lack capacity</li><li>Knowledge of modernisation agenda within the NHS and social care</li><li>Report writing skills</li><li>Teaching patients, carers and colleagues</li></ul><h3>Special Circumstances</h3><ul><li>Valid and current UK driving licence and a vehicle with Business Insurance to travel to locations throughout Buckinghamshire</li></ul><h3>Disclosure and Barring Service Check</h3><p>This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary to submit a Disclosure to the Disclosure and Barring Service to check for any previous criminal convictions.</p><h3>Salary</h3><p>£39,959 to £48,117 a year, pro rata.</p></div>
