Job summary
We're seeking experienced Registered Nurses to join our Bank Staff team in a fast-paced, non-patient-facing role supporting urgent care coordination across Somerset NHS Foundation Trust.
Key Responsibilities:
* Work flexibly across a 5-day rota within a 7-day service, including weekends and bank holidays
* Act as a Clinical Navigator in the Somerset Hub, triaging urgent referrals and coordinating care across primary, community, and secondary services
* Support discharge planning and reduce delays by liaising with multidisciplinary teams
* Provide central liaison between Rapid Response, Rehabilitation, Social Care, and referring clinicians
* Contribute to bed management and work closely with site teams and the Discharge Liaison Team
* Support remote monitoring of Hospital at Home and COVID-positive patients
* Assist in the development of the Somerset Falls Service, triaging and directing calls to Neighbourhood Hubs

* Registered Nurses (RGNs) with hospital experience
* Strong communication, clinical decision-making, and autonomy
* Flexibility to work across Somerset localities and adapt to changing priorities

Main duties, tasks & skills required
To provide and maintain a high standard of evidence based, patient centred care. To demonstrate good interpersonal skills and to develop skills required within the speciality, whist reflecting the philosophy of care.
To deliver patient care on a daily basis, using the systems in place in the department for a multi-disciplinary approach to patient-centred care delivery, which builds on the philosophy of greater involvement of patients and their relatives/carers by using the Trusts patient and public involvement strategy.
