Caerphilly County Borough Council is seeking an experienced and dedicated Social Worker to join our 16 Plus Team. This is a fantastic opportunity to work with young people transitioning into adulthood, ensuring they receive the right support to achieve independence and stability. You will play a key role in supporting care-experienced young people, providing high-quality assessment, care planning, and intervention. The role involves working closely with multi-agency partners, advocating for young people, and ensuring they have access to education, training, housing, and emotional well-being support.
Responsibilities
* Provide high-quality assessment and care management for young people requiring ongoing intervention.
* Review care plans within statutory timescales, ensuring they align with the young person's needs and aspirations.
* Work in partnership with education, housing, health, and support services to promote positive outcomes.
* Offer guidance and practical support to young people, empowering them to achieve independence.
* Represent the service at court hearings, panel meetings, and multi-agency forums.
* Ensure effective multi-disciplinary collaboration with partner agencies.
