An exciting opportunity has arisen for the position of Advanced Clinical Practitioner within UHMB Community Rapid Response Teams, incorporating 2-hour Urgent Community Response and Virtual Wards.
We have three bases at Furness General Hospital, Heysham Primary Care Centre, and Murley Moss, Kendal. The specific base will be discussed during the recruitment process.
The post holder will support the development of the expanding Rapid Response Service to deliver the mandates of the 2-hour Urgent Community Response and Virtual Wards, aiming to improve out-of-hospital care and support patients in their own homes.
The role involves working with colleagues and partners across the health and social care system to provide evidence-based, holistic care to patients requiring Virtual Wards or Urgent Community Response, as part of a Rapid Response Intervention.
This position combines direct, higher-level clinical care with education, management activities, and governance responsibilities. The post holder is expected to build collaborative relationships within the local health community.

The role is aligned with the HEE Multi-professional Framework for Advanced Clinical Practice (2017), emphasizing all four pillars: Clinical, Leadership/Management, Education, and Research/Audit.
The post aims to assist in developing a comprehensive Rapid Response and Admission Avoidance Service in the community for patients experiencing an acute illness, serving as a national exemplar for this role's potential.
Responsibilities include providing expert practice, exercising specialist professional judgment, and managing a caseload referred by GPs, consultants, and other healthcare professionals.

We operate from three main hospitals: Furness General Hospital in Barrow, Royal Lancaster Infirmary, and Westmorland General Hospital in Kendal, along with several community healthcare premises including Millom Hospital, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and RLI offer a range of hospital services, including Emergency Departments, Critical/Coronary Care, and various specialist services. WGH provides general hospital services and an Urgent Treatment Centre for non-life-threatening conditions.
All sites offer planned care such as outpatient services, diagnostics, therapies, day surgery, and inpatient care, alongside community outreach services across Morecambe Bay.
