This is an exciting opportunity to work as part of our "Outstanding Team" in the Children's Community Nursing Team caring for children across Cambridgeshire.

We are looking for a Health Care Support Worker to join our dynamic team, based out of The Oak Tree Centre in Huntingdon and The Peacock Centre in Cambridge. The team cover the whole of Cambridgeshire delivering quality care to make a difference to children and young people requiring nursing care in their homes or other community settings.

We have a dynamic team who facilitate early discharge of children from hospital as well as prevent hospital admissions and ED attendance, and are looking for enthusiastic, passionate and motivated Health Care support worker to join our team.

To work within the Children's Community Specialist Nursing Team in delivering care to children with acute and complex healthcare needs in the community. This may also include children who are
nearing the end of their life.

To deliver specific nursing interventions, promote transition to adult services and social and play tasks. and carry out duties to ensure the smooth running of Community Paediatrician clinics in the child development centre.

The post-holder will also be expected to undertake some clerical duties acting under direction of qualified professionals whilst working alongside other staff to provide both a quality and child friendly service

Detailed job description and main responsibilities


Main Duties and Responsibilities

To deliver care to patients, carers and their families adhering to the philosophy of family centred care.

To use a range of verbal and non-verbal communication tools to communicate effectively with patients and colleagues to progress their care. This may include patients who have cognitive impairment or difficulty communicating.

To act as a chaperone for vulnerable children and young people as per Chaperone policy.

Promote the health, comfort, dignity, privacy and quality of life for children and young people. To empathise with the patient and carer. To be sensitive to individual patient needs, expectations and cultural beliefs.

To work in conjunction with the Paediatrician to promote and facilitate transitions to adult services

To provide distraction and assist in helping children and young people with their coping strategies undergoing any procedures that they may find distressing.

Keeping the consultation and assessment rooms and other areas used by the children, young people and their families, or used by the staff on behalf of the patient, clean and orderly.

Ensure first line resuscitation equipment maintained in line with trust policy.

Assist in maintaining a clean and safe environment, disposing of clinical waste, rubbish and soiled linen appropriately and in the preparation of examination areas.

To carry out administrative duties as required e.g. photocopying / faxing / filling out lab forms / checking stationary stocks.

To ensure that all duties are carried out to the highest possible standard.

To work within the multidisciplinary Children's Community Specialist Nursing Service under the supervision of the senior case holders in order to provide individualised care to children and young people with complex and acute health needs, as specified by the child's protocols and in line with trust policies.

To provide care in the community to children and young people with complex and acute health needs for which training and competency assessment will be required.

This may include:

All procedures relating to naso-gastric feeding and care
All procedures relating to gastrostomy/jejunostomy
Finger prick bloods
Phlebotomy
Wounds and dressings

Communication

To participate in ensuring effective communication is maintained within the team through attending regular team meetings, clinical supervision and 1 to 1 with line manager.

To complete accurate client records in line with trust policy, using electronic 'System 1' records and home records.

To report on child/young person's progress to the clinical team at meetings or at shorter intervals, if appropriate

To promote inter-agency working (Health, Social Services, Education and Voluntary Sector) in order to utilise all available resources in the development of projects for example child/parent held records for children with special needs.

People Management

Maintaining own personal development and requirement to take part in appraisal process.

Clinical and Practice Governance

Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.

To maintain a safe environment including storage and disposal equipment.

To receive and participate in child protection supervision provided by qualified staff

To identify training needs and participate in personal appraisal, objective setting and review.

Participate in approved research projects as appropriate

Participate in Team Development Days

Any data that is taken/shared as part of a phone call or transported, faxed, or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies.

The post holder must adhere to the Trust risk assessment and risk management processes

The post holder must adhere to infection control policies and procedures

Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust.

The post holder must participate in clinical and safeguarding audits as required

Provide patients and their families /carers with information on standards they should expect from the team

Participate in Clinical & Safeguarding supervision on a regular basis

The post holder is required to participate in relevant emergency preparedness process for their team.
